First Visit
When you arrive for your first visit, please be prepared to complete all insurance and health information forms that will allow us to begin your dental treatment. We will ask you to fill out several forms that will get you acquainted with our office.
Your first dental visit will consist of a comprehensive exam which includes a review of your medical and dental history, radiographs of your teeth and supporting structures, a soft tissue examination which can include a Vizilite oral cancer screening (see link), a review of proposed treatment options, and financial arrangements. If you have had recent radiographs taken at another office, it can be helpful to request that they be sent to our office prior to your visit. You can download the appropriate forms needed for the first appointment from this website. Please fill them out prior and bring them with. In general, a cleaning will not be part of your first visit unless requested at the time you schedule your appointment. We look forward to meeting you. Please call if you have any further questions.
